To help our patients without insurance maintain their oral health we are offering an In House Patient Membership Plan. This membership is for patients without dental insurance and/or for those who prefer a simpler, more transparent approach to care. This plan focuses on prevention, clarity, and ease, without the limitations of traditional insurance.

Membership Terms & Conditions
- Membership is valid for 12 months from the date of enrollment
- Benefits must be used within the active membership period and do not carry over
- Membership should be renewed annually without interruption to maximize your oral health care benefits. If membership lapses, I may be required to complete an exam and/or cleaning at standard fees before resuming plan benefits
- This plan is not dental insurance and cannot be combined with any insurance, discount or payment plan.
- Membership fees are non-refundable after first use
- Benefits are valid only at Northgate Dental Center
